Plot Synopsis
The film begins sixteen years after the events of the original film. After the death of Marcus Aurelius, Rome is now governed by twin emperors, Geta and Caracalla, both of whom are vicious and corrupt. We then meet Hanno, or Lucius Verus, godson of Marcus Aurelius, who has taken the alias Hanno after moving to the North African Kingdom of Numidia with his wife Arishat.
Soon, the Roman Army, led by General Acacius, invades and conquers Numidia, enslaving Lucius and murdering Arishat. Lucius, along with the other slaves, are shipped to Ostia, where the Romans orchestrate a mock gladiator battle against feral baboons to advertise slaves as potential gladiators. Lucius kills a baboon and this impresses the stable master named Macrinus, who then promises Lucius an opportunity to murder Acacius if he wins enough fights in Rome.
Acacius then makes his return to Rome as a war hero, while Geta and Caracalla arrange a victorious gladiatorial games in the Coliseum in his honor. Acacius has become disillusioned with the idea of war, so Acacius requests a vacation along with his wife, Lucilla, who is the daughter of Marcus Aurelius and the mother of Lucius Verus. The emperor's refuse this request, and highlight their plan to invade India and Persia instead.
Later, Senator Thraex throws a party for the emperors and then arranges yet another gladiatorial duel as their entertainment. Lucius Verus wins the duel, which makes Geta question his origins. Lucius recites a verse from Aeneid by Virgil, which reveals his Roman education. Lucius is attended to by the gladiator’s physician, Ravi, who treats his post battle wounds and injuries. The two men become fast friends.
The way Lucius wins fights is by emulating the legendary gladiator Maximus, which causes Lucilla to recognize Lucius as her son, whom she purposefully sent away from Rome as a child to protect him from rivals and their game of thrones. Lucilla sneaks away to attempt to reconnect with Lucius, but he rebukes her attempts at reconnection, citing her living in luxury while he is forced to flee Rome, as well as the death of Arishat at the hands of her new husband.
Later, during a naumachia at the Colosseum, Lucius leads the gladiators to victory and shortly after fires a crossbow at Acacius as he watches on, but he fails to murder him. Lucius eventually learns that his friend Ravi was a former slave who also won his freedom in the Colosseum. Ravi then shows him the shrine to the legendary Maximus, which has preserved both his armor and sword. Acacius and Lucilla then conspire with Gracchus and Thraex, two senators, to overthrow the emperor's Geta and Caracalla and restore the Roman Republic. Acacuus informs Lucilla of his intention to also rescue Luicus from slavery. But Thraex, who owns Macrinus money, informs him of the coup attempt. In order to gain imperial favor, Macrinus advises Geta and Caracalla to arrest Acacius and Lucilla for treason.
Later, Macrinus arranges Lucius and Acacius to battle head to head in the Colosseum. Lucius defeats Acacius fairly easily, but fails to murder him. The Praetorian Guard then execute Acacius at the command of the two emperors, which causes the spectators to riot. Macrinus questions Lucius’s refusal, which Lucius justifies: to make Rome a better place. Macrinus disagrees and later visits Lucilla in her cell. He reveals that he was once a slave during the tenured rule of Marcus Aureloius, which made him vow to avenge himself by becoming emperor and therefore ending the Imperial Dynasty.
Later, Macrinus manipulates Caracalla into murdering Geta, which he does. Carcalla then names his pet monkey as his consul, which causes Macrinus to persuade the Senate to overthrow Caracalla. He then manipulates Carcalla to utilize Lucilla as bait within the Colosseum. This causes Lucius to defend his mother Lucilla against a horde of attackers. Macrinus is hoping the deaths of Lucilla and Lucius will be enough to cause another uprising, which will end with the Senate executing Caracalla to defuse the protests.
Lucius and Lucilla, now reconciled, unite to defeat Macrinus. Lucilla also reveals that Lucius's father is Maximus, and she bestows upon him her father’s ring, which also belonged to Maximus and Acacius. Lucius then sends Ravi along with the ring to request military aid from Acacius’s legions somewhere outside Rome. Then, Lucilla is brought into the Colosseum along with the senators Gracchus and Thraex. Lucius goes to rally the other gladiators hoping to inspire a revolt against their fellow enslavers. Now armed with his fathers sword and armor, he leads said group of gladiators to rescue Lucilla from execution. Gracchus is murdered and Macrinus kills Caracalla as well as Lucilla, whom he kills with an arrow. Macrinus flees but Lucicus is in close pursuit.
Finally, Macrinus and Acacius armies coalesce outside of Rome. In order to prevent battle, Macrinus and Lucius duel. Though Macrius almost kills Lucius, his mother’s voice comes to him, which inspires him to take the upper hand and kill Macrinus. Lucius reveals his true identity to the entirety of Rome, informing the armies that he is the rightful Imperial heir. He manages to convince the two armies to build a united Rome together and the screen fades to black.
