Sam, who suffers from terminal cancer, lives in a hospice center in the middle of New York City with her cat Frodo. Reuben, a care worker at the hospice center, tries to get Sam to join their special group outing to the island of Manhattan, which she reluctantly agrees to. During their special outing, huge unidentified meteor sized objects suddenly begin crashing into the city and soon after the meteor shower, the arrival of gargantuan extraterrestrial creatures begin their terrifying attack.

Sam is knocked unconscious during the commotion but later awakes inside a theater only to find her cat, Frodo, Reuben, and other survivors of the alien attack. The group is entirely silent with a fellow survivor named Henri signalging Sam to not make any discernible sound. Soon after, announcements from circling military helicopters desperately urge all citizens to remain quiet until they can begin making their rescues. They inform the city that all bridges in and out of the city have been destroyed in order to prevent the creatures from leaving the island. It is here when one of the survivors in the theater begins to panic and Henri, trying to keep the man calm, accidentally kills him in the process.

Ruebuen is slain by one of the creatures and this immediately inspires Sam to leave for Harlem. As she begins her trek, the military coup announces that they are preparing boats for evacuation to leave the south side of the city. The announcement sends the entirety of the city into a frenzy but the sheer volume of them moving together in tandem alert the creatures who attack the entirety of the city and in turn cause a mass stampede. Sam attempts to flee but gets separated from Frodo.

We then meet a man named Eric, a law student, who encounters Frodo from an underwater subway entrance. Froddo eventually leads Eric back to Sam and Sam tries to convince him to join the others at the evacuation point but Eric instead follows Sam to her apartment, entirely shell-shocked and traumatized. Sam is attempting to locate her pain medication, as the cancer is ravaging her body. The two eventually agree to travel as a pair, trying to avoid the creatures as the two make their way back to Harlem. Eric manages to locate an abandoned pharmacy and loot around for Sam’s pain medication. It is here where Sam reminisces about her late father, whom she used to watch perform at a jazz club and afterwards would go out for pizza, something Sam longs to do before she dies from cancer. Eric attempts to fulfill Sam’s wish.

The two become aware of two rescue boats nearby, both ready to escape the city. They make their way towards the boats, but a mob of creatures begin to fester nearby, lurking around the area. Sam hands off Frodo to Eric while she smashes car windows in an effort to distract them. Eric, who is now clutching Frodo, jumps into the water and is pulled up into the boat by Henri. It is here where Eric finds a note that Sam left for him, thanking him for his companionship and care, telling him to take care of Frodo for her.

We then see Sam, walking into an empty street, with headphones plugged into an AMFM radio which is playing “Feeling Good” by Nina Simone. Sam stands in the middle of the street, removes the headphones from the radio, and smirks, patiently waiting for their arrival. The creature looms directly behind her as the screen fades to black.
